Incident investigation - use of HSG245 - your experience.

In order to learn more from incidents we are reviewing our approach to investigation.

We are adopting the HSE document "Investigating accidents and incidents" (HSG245).

Has any member formally adopted this approach and if so what was the outcome, are there any lessons to share?

Has any member decided not to use HSG245 and adopted a different "off the shelf" methodology? If so what influenced this decision?
