Communicating safety lessons to those at the 'coal face'.

Our company works across a number of potentially high hazard sites. We employ large numbers of operational and maintenance workers and whilst much of the work they do is routine and they have done it all before, we still get the accidents.

We are looking hard at our communications processes and realise we are not good at communicating lessons and warnings to those outside the normal communications loop of email, web sites etc ie. those at the coal face who are outside the reach of the electronic world , and as a consequence the messages are not getting through to the people who matter.

Can any company for whom this sounds familiar, share practices they have successfully introduced in order to improve communication with those to whom the accidents happen? In other words, how do we get the right information, to the right people at the right time to allow them to mitigate the hazards.
