Permits - looking for improvement opportunities

This is a long and detailed question. Please answer where you can -

We are currently reviewing our permit to work Procedures, including general work , hot work and confined space entry permits. We are interested in learning from other companies as to how they manage these.

In particular:
  1. What types of permit/authorisation to work do you have in your workplace?

  2. Can you share with us a copy of your permit procedures and permit documents?

  3. Do you write permits for all work activities?

  4. How do you identify what tasks require a formal permit or a different level of authorisation before commencing work?

  5. How do you ensure that permits are returned and their status is communication to the permit issuer before they are signed off/closed?

  6. What criteria to you have for allowing permits to be "live" for more than one day or beyond the permit issuers working shift?

  7. What process do you have in place for hazard identification/assessment as part of the permit issuing process?
