Delivered competence - assurance and feedback

Given that many of the serious accidents that caused fatalities over the past decade have identified shortcomings in procedural compliance,

1. How confident are you that your procedures for ensuring that work is undertaken safely are both workable and function in practice?
2. Does your competency assessment include verifying the actual delivery / completion of expected actions, ie. beyond checking mere understanding?
3. What feedback mechanisms do you use to correct deviations from procedures when they occur?
4. How do you provide assurance to your board that compliance is on target?
