Driving at work - eye tests and maintenance

How far do members go in ensuring employees who drive on company business are safe in terms of:

1. Eye tests: do you remind people to have an eye test, make it mandatory and if so, do you pay for it?

2. Maintenance and servicing: Do you seek confirmation that cars are maintained and serviced correctly or seek evidence.

We have lease car drivers and "casual drivers" who use their own vehicles. We would be interested to hear what standards members apply for each.
