Financial incentives for good H & S performance.

Does any SHEiiBA member use a financial incentive / bonus / reward scheme for good H&S performance?

If so, at what level of management-eg Board, Executive, Plant etc - does it apply?
Are the amounts awarded based on either:-
i. a percentage of salary?
or
ii. a percentage of bonus?
If it is bonus based,
iii. what percentage of salary is the full bonus?
iv. What percentage of the bonus is the H&S reward scheme?
v. If it is salary based - what percentage is based on H&S performance? If you have another reward system, please describe it briefly.
