Drug and Alcohol Polices and responses to infringements

We are currently in the process of reviewing our Drug and Alcohol Policy and we would like to ask if any members would be willing to share theirs.

We are particularly interested to hear what measures companies take to deal with D&A policy infringements and whether or not you relate these to the employee's role i.e. do your actions differ for those undertaking safety critical roles to those in non-safety-critical roles?
