Absence Reporting - signposting for further intervention

We are planning to introduce an in-house, single point of contact for the reporting of sick absence - a single phone number for employees to call on the first day of absence.

The purpose is to measure and control absence more effectively and to signpost for further intervention e.g. industrial injuries will be routed to H&S, back pain to our physio, etc.

We are not looking for an algorithm/protocol to analyse the absences but for a "process flowchart" for the recording and distribution of information in a way that preserves medical confidentiality but ensures that the right people get appropriate details of the cases in which they can usefully intervene.

Is anyone else doing this and if so are they prepared to share their methods?
