Health and safety in annual reports

The Health and Safety Commission are saying we should publish in our annual reports information regarding the following:

a: Significant risks faced by employees and others and systems in place to control those risks.
b: Health and safety goals and progress towards achieving them.
c: Cases of ill health related to work or made worse by work.
d: Number of days lost due to injury or ill health and costs associated with that.
e: Number and detail of enforcement action against the company.
f: Number and detail of prosecutions and fines imposed.

If you publish details of any of the above in your company annual report, could you please reply by listing them by letter ( ie: b,d,f).
