HACCP standards. Looking for a checklist / auditing questionnaire

We are to audit a small canteen for food safety, and were wondering has any member developed a checklist / auditing questionnaire to check for compliance with hygiene HACCP standards, ie chilled food temperatures, food rotation, and hot water provision etc, etc,

Also have any members any Food hygiene rules or handbooks for giving to employees after their initial training.
