Confined space entry procedure and rescue - Good Practice

A member is currently revising their confined space entry procedure and wish to incorporate current good practice.

What specific controls are used to allow a rescue from a horizontal confined space such as boiler? Is a body harness and rope always mandated, where for instance the access point is restricted?
