Private driver insurance/car road-worthiness

What checks/systems does your company have in place to ensure that:

a:staff who drive their own cars on company business have adequate insurance.
b: these cars are roadworthy or does the onus rest with the owner/driver?
c: When a car > 3 years old, are companies happy to rely on the MOT
certificate as evidence that the car is roadworthy ?
