Implementing accident reporting and analysis software

Would any member be able to share their experience of implementing an accident reporting and analysis software system in a complex business? We are exploring options for a system to encompass the Group and would value being able to understand how this was approached to achieve a successful outcome. Key questions for us are around:

1. Implementing across national boundaries and languages.
2. Implementing through complex management structures.
3. Implementing in a business of diverse activities, cultures and risk profiles.
4. Implementing in a business where process safety / asset risk is a significant element of the risk profile.
Our interest is on the approach and process followed rather than which service provider was used.
