Communicating safety decisions to staff - Recording

We have had incidents of people claiming to be unaware of key safety decisions made by senior management and would be interested to know what other organisations do to record that such decisions have been fully communicated to all relevant staff and contractors. Note we are a reasonably large organisation with >1000 people on one site.
